Relapse on Ocrevus

Hi all, I have RRMS, and have had four rounds of Ocrevus infusions. Within the last fortnight, I think I have had my first relapse since beginning Ocrevus treatment. I now how spasticity on my right-hand side. Does anyone know if there are any practical next steps if you are already on a high-efficacy treatment?

@mgriffiths5 , I know nothing of your background, but how long ago did you have the last Ocrevus infusion? But, just generally, all Disease Modifying Therapies (DMTs) look to prevent progression by reducing the severity and frequency of future relapses. This means that all residual damage from previous relapses is still present. On occasions, we may put pressure on ourselves, e.g. by over-doing it, etc., and this will bring some of the old damage to the fore. So, my point is whether this spasticity on your right hand side is an old symptom reappearing, which suggests residual damage. Otherwise, if it's a new symptom, and it lasts longer than 24 hours then it may be new relapse activity which should be reported to your Neurologist.

At any rate it seems like a follow-up with your neuro would be a good idea. If I have an active inflammation/lesion (determined by MRI) I am usually put on a prednisone protocol but I suppose different Drs do different treatments depending on the patient/symptoms, etc. I'd let them know... My Dr has suggested that I consider Ocrevus as he feels I may be transitioning to SPMS but it may also be residual nerve issues from hip surgeries and that will be the focus of my appts coming up at Mayo.
