Progressive MS
i’ve been coping with progressive MS for over 20 years. i’ve been on 3 different disease modifying therapies over that span. When does primary turn to secondary? i’ve never been satisfied with broad descriptions of this diseassesincd it really does seem like e every experience is unique.
PPMS is the least frequent type if MS (~10%). It's the one where progression does not remit. So it gets worse and stays worse. Of course, you'll have worse days depending on things like heat, low level infection (dials up your immune system - never a good thing with an autoimmune condition) but in general progression is in one direction. Secondary Progressive MS is when Relapsing Remitting MS stops remitting. TL:DR - PPMS will not become SPMS
These categorisations are quite arbitrary, since they were invented in order to break MS up to access funding for a clinical trial. That said, PPMS can't become SPMS since SPMS is considered to be an evolution of RRMS. I'd recommend reading up on smouldering MS, which is basically PPMS in slow motion. A lot of neurologists now believe that most pwMS have SMS, rather than the other types. Check out Prof Giovannoni.
