Disability and CV

I have a question. Not only do I have MS recently diagnosed, I also have a brain injury caused by a ruptured brain aneurysm which left me with some complications. I want to get back into work. I’m my cv should I say I have this disability in my CV or not. I’m from the UK and I want a job at a council.
