DVLA Forms | Driving with MS

Hi all, Just looking at the forms for declaring RRMS to the dvla and under GP details it says “date of last seen for this condition” I haven’t been seen by my GP for this condition since I was diagnosed (back in Feb 2015) and that was to get a sick note extension at the time. I did get a few sick note extensions for relapses in 2019 (before I started on tysabri in 2020) but I’ve mostly had contact with my consultant. I have also changed GP’s since then too. The MS itself is well controlled on tysabri, I have no mobility issues and require no mobility aids or assistance. Before I go off on a ramble, what would the advice be for putting in last date seen by gp? Has anyone else had any experience with the DVLA and MS declarations? Any advice or help will be greatly appreciated. Thanks!

I put on mine under that section “ haven’t seen gp for this condition “ which I haven’t . Sent mine off 31 st Jan still not heard anything from them yet . Was it a CN1 form ?

@LisaJ45 ok, will put that down too. yeah it was a CN1 form, I know it can take +8 weeks for them to even respond with a decision but you can still drive during that time :)
