Mayo told me they send MS patients with low immunoglobulin to Immunology for an infusion to raise the levels back up. See if you have a similar option near by.

I’ve been monitoring mine on Ocrevus. My immunoglobulins are low but my immunoglobulin g actually increased last draw and my doc says that one is very important. I’m going to keep monitoring and then come up with a plan of “how low is too low” and have macenclad in my back pocket if I need to switch. You can get infusions to boos immunoglobulin levels, but something happens where they get too low and won’t rebound even with monthly infusions and that sounds like hell so I am keeping an eye on it. What is your experience?
