Handy cap

How do I get a handy cap flag for my car? My Dr already filled out the form but I still have no idea on where I go?

If your doctor filled it out, they should've sent it in for you. If not, I'd ask your doctor where you send it in to if there's not an address on the paperwork. If they have sent it in, it'll come in the mail.

It needs to go to your local dmv office. You could start by checking the dmv website for your state..
