Ocrevus/Kesimpta vs Tecfidera?

Hey there! I was diagnosed with RRMS in July of 2022 and will be starting treatment soon. My neuro suggested either Ocrevus or Kesimpta and I went for Ocrevus. After I had chosen (and all the documentation went through insurance, etc), I got a call from the neuro telling me she reviewed my medical history and wanted to talk about the following: - I have a sister who had breast cancer at 29 → she told me both Ocrevus and Kesimpta have been shown to increase the risk of breast cancer in patients taking them. - She told me she could switch me to Tecfidera, which is not as effective and could really do a number on my stomach for a few months. Honestly, I hate taking pills and I don't like the idea but if this is the better choice, then I would do it. So I guess my question would be: is there anyone here with a similar situation? Do you have a close relative/history with breast cancer? What did your neuro suggest? To anyone taking Tecfidera - are the side effects bad really awful on your digestive system? For how long? I don't feel confident enough to make this decision by myself and if I look for info on the web, I will find pros and cons for both sides. I am not a doctor, I have no idea what to do and my neuro told me the ball is in my court :( Any advice would be much appreciated!

Hey there, sorry this is a tough decision. Apparently the breast cancer finding in one clinical study involving ocrevus was a statical anomaly in one study so there is not yet a proven link. However since Kesimpta and Ocrevus are depleting certain B cells that could perhaps mean that in the long run there is a greater risk of developing malignancies although that’s not yet been shown as far as I’m aware. I have my discussion about meds on Friday and can update you in case my neuro has a different perspective on O and K.

Techfidera has a high chance of lowering your white blood cell count. This was the reason why I was taken off it after 18 months.
